Maintain professional competency in the areas of <br />community development, technologies, diseases, and regulations through training, workshops, <br />and speakers to address specific workforce/community needs. <br />15% Secure a grant writer with expertise in the health field; facilitates the grant-writing process as <br />needed. Research grant opportunities and local in-kind donations for community activities. <br />Identify relevant and appropriate data information sources. <br />10% Provide health-related outreach and education services to the community. Informs, <br />educates and empowers people about health issues. Collect, summarize and interpret <br />information relevant to an issue. Collaborate with community partners to promote the <br />health of the population. Inform communities about legislative changes and impacts. <br />10% Assist families in accessing health and human services. Link people to needed personal health <br />services and assure the provision of health care when otherwise unavailable. Identify health <br />needs of populations and provide public with access to information regarding community <br />resources. Facilitate community solutions to address identified issues related to health services. <br />5% Coordinate and/or participate in health fairs for local community residents. <br />4% Prepare monthly reports related to established objectives and outcomes (i.e. number of contacts <br />made, number of clients enrolled in public assistance, etc...). Evaluate effectiveness, <br />accessibility and quality of personal and population based health services. <br />1% Other duties as assigned. <br />V. LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED: <br />List the licensure, certification or registration required to perform this job: <br />State of Texas Certified Social Worker, or State of Texas Registered Nurse or Certification in <br />public health. Exnerience in nublic health or nublic administration nreferred. <br />VL KNOWLEDGE, SKILLS, AND ABILITIES: <br />Considerable knowledge of program planning and implementations, and of <br />policies and regulations governing the agencies' programs. <br />Ability to communicate clearly and effectively with community agency leaders and <br />representatives of local health foundations. <br />Skills include communication in oral and written forms of a professional caliber, <br />public speaking, use of audio/visual equipment including multi-media <br />computer equipment and organizational skills. <br />Page 5 of 7 CHRISTUS-Social Worker hd grant wi[hOUT edi[s SHOWN <br />
