Spraying Contract Bidder Disqualification Report <br />Background <br />The City of Paris issued a request for proposals (RFP) for herbicide and outdoor insect control services on <br />April 4, 2010, and received proposals from Proturf and Edko by the April 20 response deadline. At bid <br />opening the apparent low bidder was Proturf; however, upon complete review of the proposals Proturf <br />was the high bidder and also failed to respond to several minimum bid specifications. <br />Bid Deficiencies <br />The specifications Proturf did not respond to or gave a deficient response include the following: <br />2.08 The Contractor shall furnish with the bid proposal a listing of one (1) supervisor with a minimum <br />of five (5) years experience licensed in the Right-of-Way, Turf and Ornamental, and Aquatic <br />categories, and three (3) applicators with a minimum of two (2) years experience licensed by the <br />Texas Department of Agriculture or Structural Pest Control Board with the appropriate <br />categories. Contractor to furnish a listing with names, addresses and telephone numbers along <br />with photo copies of each person's Certified Commercial Pesticide Applicator Certificate and/or <br />license. <br />Proturf Response: Failed to submit the proper documentation showing proof of three (3) <br />applicators with a minimum of two (2) years experience licensed by the Texas Department of <br />Agriculture or Structural Pest Control Board with the appropriate categories. Contractor also did <br />not furnish a listing with names, addresses and telephone numbers along with photo copies of <br />each person's Certified Commercial Pesticide Applicator Certificate and/or license other than his <br />own. ) <br />6.05 Only applicators licensed by the State of Texas Structural Pest Control Board or Texas <br />Department of Agriculture will be allowed to apply herbicide or service the project. - NO <br />EXCEPTIONS. <br />Proturf Response: Bidder verbally informed City that he has three employees, but did not <br />provide in the bid response any information of the three employees and whether they were <br />licensed or not. <br />6.07 All equipment used by the Contractor for herbicide application must be inspected and licensed by <br />the Texas Structural Pest Control Board or Texas Department of Agiculture and must comply <br />with all applicable State, Federal and OSHA regulations. The Contractor shall submit a listing of <br />equipment to be used with the bid proposal as a basis for the City of Paris to determine the <br />Contractors ability to perform the work within the time frame specified. <br />Proturf Response: Bidder submitted the list of equipment but did not submit the Texas <br />Department of Agricultural license ar number with those. <br />8.00 RESPONSIBILITY OF PROPERTY DAMAGE: <br />C) Motor Vehicle Public Liability <br />City of Paris <br />4/29/10 <br />67 <br />
