<br /> <br /> 1rerracon <br /> 8901 Carpenter Freeway, Suite 100 <br /> Dallas, Texas 75247 <br /> 214-630-1010 <br /> <br /> PHASE I SCOPE OF WORK <br /> <br /> FUNGAL-BIOLOGICAL HAZARD REMEDIATION <br /> <br /> Project: Grand Theater <br /> Phase 1 <br /> 21 Lamar Avenue <br /> Paris, Texas 75460 <br /> <br /> Terracon Project No. 94107645 <br /> <br /> PROJECT SUMMARY: <br /> <br /> Terracon documented water damage, fungal growth, and biological contamination <br /> <br /> associated with pigeon infestation in the Grand Theater first floor lobby and the second floor <br /> space directly above the lobby (Phase 1). Based on the evidence of chronic moisture <br /> intrusion, visible water damage, and visible fungal growth Terracon documented regulated <br /> fungal contamination on the wood ceiling deck in the first floor lobby. Additionally, non- <br /> regulated fungal growth is present on water damaged ceiling tiles and ceiling tile debris in <br /> the first floor lobby. Biological-hazard contamination related to historical pigeon infestation <br /> is present as pigeon feces, and pigeon carcasses on the second floor above the lobby and <br /> within an interstitial building space located at the north end of the second floor above the <br /> lobby. The quantity and location of water damage/wood rot, fungal growth, and pigeon <br /> debris in the referenced locations is further described in Section 10.0, and the site diagrams <br /> in Appendix A. <br /> <br /> The remedial activities will be conducted in general accordance with the Texas Department <br /> of State Health Services (TDSHS) regulations contained in "The Texas Mold Assessment <br /> and Remediation Rules", (TMARR). In general, the remediation of non-regulated fungal <br /> growth will consist of the removal and disposal of drop ceiling tiles, applied ceiling tiles and <br /> <br /> ceiling tile debris in the first floor lobby. Regulated fungal growth on the water damaged <br /> wood ceiling deck will be removed by hand cleaning the wood substrate or by carbon <br /> dioxide-blasting of the wood substrate, and the ceiling deck encapsulated. The TDSHS has <br /> granted a project specific exemption related to "wood rot" associated with the wood ceiling <br /> deck and common second floor (Figure F-1, Appendix A) since the wood ceiling <br /> deck/second floor will eventually be removed, inclusive of the visible wood rot. The general <br /> debris and the pigeon debris that comprise the defined biological-hazard on the second floor <br /> will be removed and disposed as construction debris. After the general debris has been <br /> removed from the second floor visible fungal growth on the wood floor substrate will be hand <br /> cleaned or cleaned by carbon dioxide-blasting, and the floor encapsulated. The remediation <br /> work will be conducted within containment(s) under negative pressure with HEPA filtered <br /> exhaust.
