<br />DRAFT <br /> <br />5. <br /> <br />H. <br /> <br />I. <br /> <br />FAILING A TEST - <br /> <br />1. <br /> <br />For incoming employees, failure to consent to or pass the required controlled <br />substance and alcohol screening will be considered just grounds to void the tentative <br />job offer. Further, individuals failing or refusing to take a controlled substance and <br />alcohol screen after a tentative job offer will not be reconsidered for employment for <br />a period of twelve (12) months. <br /> <br />2. <br /> <br />Current employees who fail a controlled substance or alcohol screening required in <br />accordance with this Policy shall be subject to disciplinary action, up to and <br />including termination. <br /> <br />REFUSAL TO TEST - An employee who refuses to undergo a controlled substance or <br />alcohol screening in accordance with this Policy shall be treated as if the employee failed the <br />required test. <br /> <br />CONFIDENTIALITY OF RESULTS - All controlled substance and alcohol screening <br />results shall be the private and confidential property of the City of Paris and will not be <br />shared with anyone except the employee, City supervisory staff with a demonstrated need to <br />know such information, or as required by a court order, D.O.T. mandates, or to legally <br />protect the City. <br /> <br />SECTION 17.08 - Controlled Substance and Alcohol Screenine Procedures <br /> <br />The following procedures will be observed in the administration of the controlled substance and <br />alcohol screening process: <br /> <br />A. <br /> <br />EMPLOYEE RESPONSIBILITY - When required to submit to a controlled substance and <br />alcohol screening the employee will: <br /> <br />1. <br /> <br />present identification containing a photograph and will, if physically able, complete <br />and sign a test authorization form and other paperwork required by the medical <br />facility. <br /> <br />2. <br /> <br />provide a breath sample in accordance with established procedures as required by the <br />medical facility. <br /> <br />3. <br /> <br />provide the medical facility a urine specimen to be divided into two (2) samples. <br /> <br />4. <br /> <br />initial each specimen collection bottle or other similar documentation to accompany <br />the specimen. <br /> <br />53 <br /> <br />Rev. 10/27/04 <br />
