<br />DRAFT <br /> <br />B. <br /> <br />5. <br /> <br />along with the attending nurse, initiate "chain of custody" by signing and dating the <br />Chain of Custody Form and witnessing the processing of the sample containers. <br /> <br />6. <br /> <br />where the employee's injuries prevent ordinary sample techniques, submit to a <br />substitute sample technique such as blood sampling and testing. <br /> <br />MEDICAL FACILITY RESPONSIBILITY - The Medical Facility will: <br /> <br />1. <br /> <br />perform alcohol testing for all employees in accordance with city, state, and federal <br />guidelines. Breath alcohol testing will be performed with certified equipment by a <br />certified technician. <br /> <br />2. <br /> <br />perform controlled substance testing for all employees in accordance with city, state <br />and federal guidelines. <br /> <br />3. <br /> <br />immediately implement the steps necessary to ensure test confirmation when initial <br />test results are positive. <br /> <br />4. <br /> <br />perform the collection of an employee's urine sample for controlled substance <br />screening and, when applicable, the actual initial controlled substance test following <br />the collection of specimen steps listed below: <br /> <br />a. <br /> <br />place one (1) specimen with the original copy of the Test Requisition Form <br />in a tamper proof chain of custody pouch if forwarding to another facility for <br />testing. Chain of custody will be maintained internally if the medical facility <br />performs the initial screening. <br /> <br />b. <br /> <br />when necessary, release the patient's first specimen to a courier to deliver <br />directly to a nationally certified testing facility. <br /> <br />c. <br /> <br />retain the second urine specimen (labeled as original specimen) for a <br />minimum of two (2) weeks as a backup in the event of contamination of the <br />first sample in transit or any other need for a second examination of the <br />patient's specimen. <br /> <br />5. <br /> <br />for all non-safety sensitive positions, complete an initial controlled substance <br />screening of the specimen. The initial test will be done in accordance with state and <br />federal guidelines. Should this test be positive, a confirmation test, the Gas <br />Chromatography/Mass Spectrometry (Ge/MS) test, will be completed by a nationally <br />certified testing facility. <br /> <br />54 <br /> <br />Rev. 10/27/04 <br />
