<br />DRAFT <br /> <br />c. <br /> <br />6. <br /> <br />ensure that the following substances are included in the controlled substance testing <br />process: <br /> <br />Amphetamines <br /> <br />Cannabinoids (THC) <br /> <br />Phencyclidine (PCP) <br /> <br />Barbiturates <br />Cocaine Metabolites <br />Propoxyphene <br /> <br />Benzodiazepines <br />Opiates <br />Inhalants <br /> <br />7. <br /> <br />where the employee's injuries prevent ordinary sample techniques and testing, initiate <br />a substitute sample technique, such as blood sampling and testing. <br /> <br />8. <br /> <br />when conducting testing for non-safety sensitive positions, contact the appropriate <br />City representative as soon as possible, but in no event later than twelve (12) hours <br />after receipt of the employee's specimen, with initial test results and, if required, <br />within forty-eight (48) hours after receipt of the employee's specimen by the testing <br />facility that will administer the test confirmation. <br /> <br />9. <br /> <br />when coordinating testing for safety sensitive positions, contact the appropriate City <br />representative within forty-eight (48) hours after receipt of the employee's specimen <br />with initial test results and, if required, within forty-eight (48) hours after the request <br />for test confirmation. <br /> <br />10. <br /> <br />at all times, maintain confidentiality of test results. <br /> <br />PERSONNEL DEP ARTMENT RESPONSIBILITY - The Personnel Department will: <br /> <br />1. <br /> <br />select the medical facility or facilities which will conduct collections and screenings. <br /> <br />2. <br /> <br />inform an incoming employee, or when not required to be done by the employee's <br />supervisor, a current employee, of the City's controlled substance and alcohol testing <br />procedures and require that a CONSENT FORM FOR CONTROLLED <br />SUBSTANCE AND ALCOHOL TESTING be read and signed prior to sending the <br />employee for testing. <br /> <br />3. <br /> <br />receive the screening results from the medical facility. <br /> <br />4. <br /> <br />resolve concerns regarding the medical results of a positive test by: <br /> <br />a. <br /> <br />contacting the Medical Review Officer and clarifying any medical questions, <br />disagreement, or uncertainty. <br /> <br />b. <br /> <br />initiating any required testing of the employee's second specimen. <br /> <br />55 <br /> <br />Rev. 10/27/04 <br />
