<br /> <br /> <br /> <br /> <br /> <br /> (11) Any other conditions may be included as <br /> deemed appropriat:e by the City to ensure <br /> compliance with this article. <br /> (d) Signatory requirements. All reports or <br /> other required documentation shall be signed and <br /> certified by the authorized representative of the <br /> permit halder. The certificatiori statements shall <br /> be worded as follows: <br /> "I certify, under penalty of law, that this <br /> document and all attachments were prepared under my <br /> direction or supervision in accordance with a <br /> system designed to assure that qualified personnel <br /> properly gathered and evaluated the information <br /> submitted. Based on my inquiry of the person or <br /> persons who manage the system, or those persons <br /> directly responsible for gathering the information, <br /> the information submitted is, to the best of my <br /> knowledge and belief, true, accurate, and complete. <br /> I am aware that there are significant penalties for <br /> submitting false information, including the possi- <br /> bility of fines and/or imprisonment for knowing <br /> violations." <br /> (e) Permit duration. Permits shall be issued <br /> for a specified time period, not to exceed five (5) <br /> years. A permit may be issued for a period less <br /> thati a year or may be stated to expire on a <br /> specific date. The terms and conditions of the <br /> permit may be subject to modifications by the City <br /> during the term ot the permit as limitations or <br /> requirements are modified or just cause exists. <br /> The user shall be informed of any proposed changes <br /> in this permit at least thirty (30) days prior to <br /> the effective date of change. Any changes or new <br /> conditions in the permit shall include a reasonable <br /> time schedule for compl.iance. <br /> (f) Permit renewal. Industrial dischargers <br /> holding a wastewater contribution permit shall <br /> apply for a permit renewal one hundred twenty (120) <br /> days before their permit expires. Any desired <br /> changes in permit limits, changes in plant piping, <br /> changes in wastewater quality or pretreatment <br /> facilities made since the issuance of the last <br /> permit or additional iriformation required by the <br /> city should be a part ut the application for permit <br /> renewal. <br /> (g) Permit transfer. Wastewater contribution <br /> permits are issued to a specific user for a <br /> specific operation. A wastewater contribution <br /> permit shall not be transferable. Any succeeding <br /> owner or user shall apply for a new permit. <br /> (h) Permit modifications. Within ninety (90) <br /> days following the promulgation of a national <br /> categorical pretreatment standard, the wastewater <br /> contribution permit of users subject to such <br /> standards shall be revised to require compliance <br /> with such standard within the time frame prescribed <br /> by such standard. Where a user subject to a <br /> national categarical pretreatment standard has not <br /> previously submitted an application for a waste- <br /> water contribution permit as previously described, <br /> the user shall apply for and submit such <br /> application within nznety (90) days after the <br /> promulgation of the applicable national categorical <br /> pretreatment standard. Compliance shall be <br /> achieved within the time frames prescribed by the <br /> standards. <br /> -18- <br />
