Report <br /> monthly to the City the total volume <br /> of waste transported and all <br /> utilized disposal sites for the <br /> previous manth. <br /> The City may cause to be <br /> impounded a vehicle which is being <br /> operated in violation of Section <br /> 34-131 and may authorize the holding <br /> of the vehicle until the violation <br /> is corrected. The City may also <br /> suspend or revoke the permit for the <br /> impropeily operated vehicle. <br /> h) A manifest system, consisting of a <br /> four-part trip ticket to document the <br /> generation, transportation, and disposal <br /> of liquid waste shall be used as follows: <br /> 1) The trip ticket books are <br /> purchased by the transporter from <br /> the City for a fee as set forth in <br /> Section 34-131(c)(6)c). <br /> 2) A transporter will complete one <br /> trip ticket for each location <br /> serviced. <br /> <br /> 3 Copy one of the trip ticket is <br /> signed by the transporter and <br /> generatar at the time of waste <br /> removal and copy four is left with <br /> the generator for their files. <br /> 4) Copy one of trip ticket is <br /> signed by disposer at time of <br /> disposal and copy two is maintained <br /> by the disposer. <br /> 5) Copy three of trip ticket (with <br /> signatures of generator, trans- <br /> porter, and disposer) are maintained <br /> by transporter. <br /> 6) All_ utilized trip ticket books <br /> containing copy one will be <br /> delivered within ten (10) days <br /> following the motith of completion by <br /> the transporter to the director of <br /> utilities of the City. <br /> 7) A copy of all trip tickets <br /> shall be maintained for a periad of <br /> three (3) years. <br /> <br /> (4) Denial, Suspension, Kevocation of Permit <br /> a) The authority may deny a permit if <br /> it is determined that an applicant has <br /> not performed the requirements of <br /> Sections 34-131(c)(1) and 34-131(c)(3) of <br /> this article and may suspend a permit if <br /> it is determined that a permittee: <br /> 1) Is not qualified under Section <br /> 34-131(c)(1) of this article. <br /> <br /> 2) Has violated a provision of <br /> this article. <br /> -38- <br />
