Type of organization, i.e., corporation, sole proprietor, partnership, etc. <br />3. When organized. <br />4. If a corporation, where incorporated. <br />5. State how many years you have been engaged in the contracting business under your present <br />firm or trade natne, and give all other aames under which your organization has operated in <br />the last 10 years. <br />6. State the name of all corporate of~'icers, or partners, or the owners, as appropriate. <br />7. List the type of construction performed within the past 12 months, i.e., home remodeling, <br />new home construction, major construction, etc. <br />8. Have you ever failed to complete any work awarded to you7 <br />9. Have you ever defaulted on a contrar.t? If so, where aud why? <br />10. List the name, address and phone number of the last three clients in which construction is <br />complete. <br />11.
