<br /> <br /> <br /> <br /> <br /> <br /> SECTION 31-40. WASTEWATER CONTRIBUTION PERMIT <br /> SYSTEM <br /> A. REQUIREMENT FOR A PERMIT <br /> <br /> Any user shall have a wastewater contribution permit <br /> prior to discharging into the City sewer system if : <br /> <br /> 1) the discharge is subject to promulgated National <br /> Categorical Pretreatment Standards; or, <br /> 2) the discharge contains pollutants in concentrations <br /> or quantities that interfere or have the potential <br /> to interfere with the operation of the sewer <br /> system or increase the costs of operation of the <br /> system, as determined by the City Engineer; or, <br /> 3) the discharge requires pretreatment in order to <br /> comply with the discharge limitations in this <br /> ordinance; or, <br /> 4) the discharge is metered at or the water usage <br /> exceeds 15,000 gallons per day; or, <br /> 5) the user has on premises liquid or solid wastes <br /> which require a state or federal permit or license <br /> for disposal. <br /> B. PERMIT INFORMATION <br /> New significant industrial users shall file an application <br /> in the form prescribed by the City at least ninety (90) <br /> days prior to connecting to the POTW. Any other <br /> non-domestic user shall file an application when notified <br /> within 90 days of receipt of notice to do so. In support of <br /> the application, the user shall submit in units and terms <br /> appropriate for evaluation, the following information as <br /> required by the Supervisor: <br /> 1) Name, address, and location (if different from the <br /> address); <br /> 2) 5IC number according to the Standard Industrial <br /> Classification Manual, Bureau of the Budget, <br /> 1972, as amended; <br /> 3) Wastewater constituents and characteristics as <br /> determined by a reliable analytical laboratory; <br /> sampling and analysis shall be performed in <br /> accordance with procedures established by the <br /> EPA pursuant to Section 304 (g) of the Act and <br /> contained in 40 CFR, Part 136, as amended; <br /> 4) Time and duration of contribution; <br /> 5) Average daily an d fifteen (15) minute peak <br /> wastewater flow rates, including daily, monthly <br /> and seasonal variations, if any; <br /> 6) Site plans, floor plans, mechanical and plumbing <br /> plans and details to show all sewers, sewer <br /> connections, and appurtenances by the size, <br /> location and elevati.on; <br /> 7) A general description of activities, facilities and <br /> plant processes on the premises including all <br /> materials which are or could be discharged <br /> (including cleanup chemicals and washdown water) <br /> which make use of, involve or incorporate any <br /> x•estricted, limited or prohibited substances or <br /> toxic pollutants. <br /> 8) The nature and concentration of any pollutants in <br /> the discharge which are limited by any City, <br /> state or federal pretreatment standards, and a <br /> statement regardirig whether or not the pre- <br /> treatment standards are being met on a consistent <br /> basis. If standards are not being so met, <br /> whether additional operation and maintenance <br /> (O&M) and/or additional pretreatment is required <br /> 22 <br />
