Irerracon <br />8901 Carpenter Freeway, Suite 100 <br />Dallas, Texas 75247 <br />214-630-1010 <br />SCOPE OF WORK <br />FUNGAL-BIOLOGICAL HAZARD REMEDIATION <br />June 4, 2012 <br />Project: Grand Theater <br />Phase 11 <br />21 Lamar Avenue <br />Paris, Texas 75460 <br />Terracon Project No. 94107645 <br />PROJECT SUMMARY: <br />The Grand Theater, Phase II Fungal Biological Hazard Remediation project is demarcated <br />in the attached drawing (Figure G-1). <br />Terracon documented water damage, fungal growth, and biological contamination <br />associated with pigeon infestation in the Grand Theater in the location identified in the <br />attached drawing (Figures F-1 and F-2). Additionally, non-regulated fungal growth may be <br />present on water damaged building materials at various locations within the contained work <br />area. Biological-hazard contamination related to historical pigeon infestation is present as <br />pigeon feces and pigeon carcasses and for the purposes of this report will be described as <br />"pigeon related debris". Pigeon related debris (PRD) is present on the majority of horizontal <br />surfaces in the building areas that comprise Phase II. The quantity and location of water <br />damage/wood rot, fungal growth, and PRD in the referenced locations is further described in <br />Section 10.0, and the site diagrams in Appendix A. <br />The remedial activities will be conducted in general accordance with the Texas Department <br />of State Health Services (TDSHS) regulations contained in "The Texas Mold Assessment <br />and Remediation Ru/es", (TMARR). In general, the remediation of regulated fungal growth <br />will consist of the removal and disposal of building materials as referenced in Section 10.0 <br />and demarcated in the drawings presented in Appendix A. The general debris and the PRD <br />that comprise the defined biological-hazard in Phase II locations will be removed and <br />disposed as construction debris, unless the material is further contaminated with asbestos <br />or lead paint as defined in Terracon's corollary Scope of Work for Hazardous Materials <br />Abatement, Terracon Project No. 94107645, dated June 4, 2012. The remediation work will <br />be conducted within containment(s) under negative pressure with HEPA filtered exhaust. <br />The remedial work conducted in Phase II building locations will require the use of full face N- <br />100 respirators. Due to the potential for elevated concentrations of biological pathogens <br />related to PRD, Terracon recommends that the use of Powered Air Purifying Respirators <br />(PAPR) should be considered. <br />Responsive ■ Resourceful ■ Reliable 1 <br />MAC # 0107/11-10-12 <br />
