Fungal-Biological Hazard Remediation Protocol-Phase II <br />Grand Theater ■ 21 Lamar Street, Paris, Texas <br />February 24, 2011 ■ Terracon Project No. 94117645 <br />Irerracon <br />Terracon has documented the presence of asbestos containing materials (ACM) and lead <br />based paint (LBP) in the majority of the Phase II building locations. The abatement of ACM <br />and LBP will be conducted prior to or simultaneous to fungal and PRD remediation. The <br />asbestos and LBP abatement specifications are provided in the Scope of Work for <br />Hazardous Materials Abatement, Terracon Project No. 94107645, dated June 4, 2012. <br />1.0 SAFETY PRACTICES <br />The contractor is responsible for all OSHA safety practices associated with worker <br />protection. In addition, the contractor shall ensure that all electrical and fire hazard safety <br />protocols are followed in compliance with typical construction regulations. It is the <br />contractor's responsibility to ensure that workers on scaffold platforms or working from <br />ladders or other elevated platforms should be safe and secure from slipping or falling. <br />2.0 ACCESS TO WORK AREA <br />A secure work area will be established. Only licensed/registered individuals qualified to <br />wear respiratory protection will enter the vacant building work area during the project. The <br />contractor will ensure that access to the work area is periodically monitored. <br />3.0 MOVEABLE/NON-MOVEABLE OBJECTS <br />Not applicable. <br />4.0 HEATING, VENTILATION, AND AIR CONDITIONING SYSTEM EQUIPMENT <br />Not applicable. <br />5.0 DECONTAMINATION SYSTEM <br />a) The Phase 11 remediation work area will be entered through the decontamination <br />system previously constructed for the ACM-LBP abatement as referenced in Scope <br />of Work for Hazardous Materials Abatement, Terracon Project No. 94107645, June <br />4, 2012. <br />6.0 CONTAINMENT/CRITICAL BARRIERS <br />a) The Phase II work areas will have the majority of the required containments and <br />critical barriers in place from the previous ACM-LBP abatement as referenced in <br />Scope of Work for Hazardous Materials Abatement, Terracon Project No. 94107645, <br />June 4, 2012. The TMARR stipulates that a vacant building with controlled access <br />does not require negatively pressurized, contained work areas, if only licensed <br />6?esponsive ■ Resourceful ■ FZeliable 2 <br />MAC # 0107/11-10-12 <br />
