Phase 11 - Scope of Work - Lead Paint Abatement Irerracon <br />Grand Theatre ■ 21 Lamar Avenue, Paris, Texas <br />June 4, 2012 ■ Terracon Project No. 94107645 <br />Second Floor - Lobbv/Restroom/Office (see attached drawinq) <br />Plaster Walls/Ceilings <br />Plaster Crown Molding <br />Metal Door, Jamb and Casings <br />Wood Door, Jamb and Casings <br />Wood Baseboards <br />Metal Window, Casing and Sash <br />NOTES: <br />a) CONTRACTOR will be responsible for site security throughout the remediation <br />project. <br />CONTRACTOR will remove and dispose of all the theater seating in each <br />theater. The seats may be cleaned and decontaminated and disposed of as <br />general construction debris. They CONTRACTOR must take special care not to <br />inadvertently remove materials containing lead paint outside the scope of lead <br />remediation. <br />b) CONTRACTOR will remove the lay-in ceiling tile system in second floor men and <br />women's restroom. These materials will be disposed of contaminated with lead. <br />c) CONTRACTOR will remove the carpet in the second floor lobby and office area <br />as contaminated with lead. <br />d) CONTRACTOR will remove the wall curtains in the first floor theaters within the <br />work area. These materials will be disposed of as contaminated with lead. <br />e) CONTRACTOR will remove the water damaged ceiling on the south end of the <br />second floor lobby inclusive of the plaster, wire mesh and metal supports. <br />fl CONTRACTOR will remove the damaged plaster walls in each work area to the <br />brick substrate. <br />g) CONTRACTOR is responsible for providing all scaffolding, ladders and/or <br />staging necessary to accomplish the work of this contract. The type, erection and <br />use of scaffolding, including provisions for fall protection and employee training, <br />shall comply with OSHA 29 CFR 1962, SUBPART L. <br />h) CONTRACTOR is responsible for Fall Protection for his employees. <br />CONTRACTOR is solely responsible for protection CONTRACTOR's employees <br />Responsive ■ Resourceful■ Reliable 2 <br />
