The <br /> contractor shall not be responsible for notification fees <br /> in their bid. The contractor shall be responsible for <br /> obtaining and filing all additional permits and notices <br /> where required by local ordinances. <br /> <br />_ 1.1.14 The contractor shall be licensed through the Texas <br /> Department of Health for asbestos abatement. All <br /> supervisors and workers involved with the project (during <br />_ prepping, precleaning, removal, etc.) shall be EPA <br /> accredited, Texas licensed and/or registered, and shall <br /> have a current medical evaluation and respirator fit <br /> tests in accordance with the OSHA requirements of CFR <br /> 1926.1101. <br /> <br /> 1.1.15 Submittals <br /> <br /> 1.1.15.1 Pre-Job Submittals: <br /> <br />-- The following items shall be delivered to the project <br /> consultant no later than Thursday May 1, 2003, prior to the job <br /> start date. <br /> <br /> a. EPA Training Certificates: <br /> <br />--' (1)Supervisor/Contractor - Asbestos Abatement (40 hr.) <br /> Workers <br /> (1)Asbestos Worker Training (32 hr.) <br /> <br /> b. TDH Licensing: <br /> Abatement Contractor License <br /> Asbestos Transporter License <br />-- Asbestos Supervisor <br /> c. TDH Worker Registration <br /> d. Medical clearance for each worker and supervisor. <br /> e. Certificates of Insurance showing City of Paris as <br /> Additionally Insured <br /> <br /> <br />
