Include review of progress with respect to <br /> previously established milestones and schedules, major <br /> problems and actions taken, injury reports, and equipment <br /> breakdown. <br /> <br /> 1.4.1.2.2 Submit copies of all transport manifests, trip tickets and <br /> disposal receipts for all asbestos waste materials removed <br /> from the work area during the abatement process. Notify <br /> the Owner's Representative at least 24 hours prior to <br /> transporting of date and time. <br /> <br /> 1.4.1.2.3 Submit daily, copies of worksite entry logbooks with <br /> information on worker and visitor access. <br /> <br /> 1.4.1.2.4 Submit logs documenting filter changes on respirators, <br /> HEPA vacuums, negative pressure ventilation units, and <br /> other engineering controls. <br /> <br /> 1.4.1.2.5 Maintain on-site results of bulk material analysis and <br /> ambient air sampling analysis and other data collected <br /> during the course of the abatement including OSKA <br /> compliance air monitoring results. <br /> <br /> 1.4.1.2.6 Post the laboratory analysis results of personal samples <br /> within 24 hours of collection. Furnish a copy to the <br /> Owner's Representative either by hand or facsimile <br /> delivery. <br /> <br /> 1.4.1.2.7 Submit results of materials testing conducted during the <br /> abatement for purposes of utilization during abatement <br /> (e.g. testing of encapsulant for depth of penetration, <br /> <br /> testing of substitute materials for adherence to <br /> <br /> encapsulated surfaces). <br /> 1.4.1.3 After abatement. <br /> <br /> 1.4.1.3.1 Submit to Owner's Representative within 10 days the <br /> project report consisting of the following: <br /> <br /> 1.4.1.3.1.1 As a minimum, NESHAP notice, project log, sign <br /> in/out, equipment certifications, record of HEPA <br /> filter changes, differential pressure logs, <br /> respirator programs, personal sample log and <br /> laboratory reports showing time weighted averages <br /> for workers in each job task with social security <br /> numbers, emergency plan, safety and hazard <br /> communication training documentation, including <br /> MS/DS log, laboratory reports of samples collected <br />4/11/03 48 <br /> <br /> <br />
