Information to Customers <br />Company will: <br /> <br /> (A) maintain a current set of maps showing the physical locations of its facilities. All <br /> distribution facilities will be labeled to indicate the size or any per[inent information that will <br /> accurately describe Company's facilities. These maps, or such other maps as may be required <br /> by the Regulatory Authority, will be kept by Company in a central location and will be available for <br /> Regulatory Authority inspection only during normal working hours. Subject to Company's right to <br /> protect the security of its facilities, Company will make available up-to-date maps, plans, or <br /> records of its immediate area, with such other information as may be necessary to advise <br /> Applicants and others entitled to the information as to the facilities available for serving that <br /> locality; <br /> <br /> (B) in compliance with applicable law or regulations, notify Customers affected by a change <br /> in rates or schedule or classification; <br /> <br /> (C) upon request, inform Customer as to the method of reading Meters; <br /> <br /> (D) inform or provide to new Residential and Commercial Customers, at the time service is <br /> requested or initiated, or provide as an insert in the first billing, a pamphlet or information packet <br /> containing the following information. This information will be provided in English and Spanish to <br /> adequately inform Customer; provided, however, the Regulatory Authority, upon application and a <br /> showing of good cause, may exempt Company from the requirement that the information be <br /> provided in Spanish. <br /> <br /> (1) Customer's right to information concerning rates and services and Customer's <br /> right to inspect or obtain, at reproduction cost, a copy of the applicable tariffs and service <br /> rules. <br /> <br /> (2) Customer's right to have his or her Meter checked without charge under these <br /> Service Regulations (relating to Meters), if applicable. <br /> <br /> (3) The time allowed to pay outstanding bills. <br /> <br /> (4) Grounds for termination of service. <br /> <br /> (5) The steps Company must take before terminating service. <br /> <br /> (6) How Customer can resolve billing disputes with Company and how disputes and <br /> health emergencies may affect termination of service. <br /> <br /> (7) Information on alternative payment plans if offered by Company. <br /> <br /> (8) The steps necessary to have service reconnected after involuntary termination. <br /> (9) The appropriate Regulatory Authority with whom to register a complaint and how <br /> to contact such authority. <br /> <br /> -72- <br /> <br /> <br />
