Applicants will be notified <br />by mail when the fee is due. <br />APPLICATIONS FOR CEIMITCATES , <br />Application shall be made on a standard application form which will be available <br />at the designated test sites on the day of the test. The application must be <br />completed in full, with attached sheets as necessary, signed, dated, and <br />submitted with the examination answer sheet.
