�l <br />524 <br />SECTION 13.04 — Department Personnel File <br />The Department Director or his designee maintains the Department personnel file. The following <br />items should be maintained in the department personnel file: <br />A. Copies of application forms <br />B. Conference notes <br />C. Copies of Payroll information <br />D. Copies of disciplinary actions <br />E. Copies of performance evaluations <br />The Department personnel file should not include medical information. All medical information <br />should be maintained in the employee's medical file which is maintained in the Human Resources <br />Office. <br />30 Revised 01 -25 -05 <br />
