<br />RESOLUTION NO. 97 -026
<br />WHEREAS, the State of Texas is a funding partner with the County of Lamar and the City
<br />of Paris for the Paris -Lamar County Health Department; and,
<br />WHEREAS, the Paris -Lamar County Health Department did heretofore enter into a
<br />contract with the Texas Department of Health, TDH Document No. 7560022067 96, which is a
<br />conduit through which state funds are received; and,
<br />WHEREAS, said contract has been amended, and it is deemed appropriate that the form
<br />of the Contract Change Notice No. 07, attached hereto as Exhibit A, be, in all things, approved,
<br />and that the City Manager, Michael E. Malone, be authorized to execute the same on behalf of the
<br />City of Paris, which is the authorized contracting entity for the performing agency, the Paris -
<br />Lamar County Health Department; NOW, THEREFORE,
<br />BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PARIS, that the
<br />form of the Contract Change Notice No. 07 to the contract between the Paris -Lamar County
<br />Health Department and the Texas Department of Health, TDH Document No. 7560022067 96,
<br />attached hereto as Exhibit A, be, and the same is hereby, approved; and,
<br />BE IT FURTHER RESOLVED, that the City Manager, Michael E. Malone, be, and he
<br />is hereby, authorized and directed to execute, on behalf of the City of Paris, which is the
<br />authorized contracting entity for the performing agency, the Paris -Lamar County Health
<br />Department, the Contract Change Notice No. 07 under the terms and conditions and in the form
<br />shown in Exhibit A, attached hereto.

<br />Resolution No. 97 -027, resolving that the "Rehabilitation and Painting of Two -Two Million
<br />Gallon Steel Ground Storage Water Tanks" Project No. 92- WSRB02 -96 contract is hereby
<br />amended by Change Order No. One, providing for a net increase in contract price of $5,000.00
<br />to modify method to correct ponding water on the roof of the north tank by removal and
<br />reinstallation of roof shell allowing for the approved installation of up to 1178 square inches
<br />of 1/4" steel plate to correct severe pitting, for a total contract price of $459,913.00, and
<br />authorizing the Mayor of the Mayor of Paris, Eric S. Clifford, to execute such Change Order,
<br />was presented. A motion was made by Councilman Boyd, seconded by Councilman Neeley
<br />for approval of the resolution. The motion carried 5 ayes, 0 nays.
