City of Paris — Construction Inspection H.W. Lochner, Inc. <br />ATTACHMENT A <br />SERVICES TO BE PROVIDED BY THE CONSULTANT <br />The City of Paris, Texas (City) is entering into a contract with a H.W. Lochner, Inc. (Consultant) <br />to provide Inspection staff to perform various types of services that include: inspect new utility <br />construction, surface improvements (including pavement, curb and gutter, sidewalk and other <br />facilities to be constructed in the Public Right -of -way), alterations and repairs to existing <br />facilities, traffic control facilities and operations, trench work (including verifying bedding <br />materials, trench backfill and pavement restoration), insure compliance with plans, <br />specifications and applicable federal, state and local codes. Responsibilities include: <br />A. Review all approved plans, specifications and documents. <br />B. Verify facilities to be inspected are constructed in accordance with applicable plans, <br />specification and state and local regulations. <br />C. Maintain construction inspection records. Utilize a web -based management program for <br />tracking of the construction progress along with communications, including memos, <br />daily reports, transmittals, and reporting. <br />D. Submit monthly progress reports. <br />E. Notify Contractors and the City of noncompliance with the contract plans and <br />specifications in a timely manner. <br />F. Coordinate with City staff regarding aspects and status of construction. <br />GENERAL REQUIREMENTS <br />The Consultant shall be responsible for inspection of the projects to ensure the specific <br />operations are conducted in accordance with the construction plans and specifications. <br />The Consultant shall assist the City in matters of policy and procedure, and generally accepted <br />industry practices. The Consultant shall identify deficiencies in the work of the contractor, its <br />subcontractors, its vendors and its consultants in the specific construction operations and <br />inform the City of these deficiencies. <br />The Consultant shall provide qualified technical and professional personnel to adhere to <br />professional standards consistent with those typically met by nationally known and highly <br />regarded construction inspection firms assigned with the terms of this agreement. <br />Progress Reporting and Invoicing. The Consultant shall submit a monthly written progress <br />report to the City. The Consultant's written progress report shall describe activities during the <br />reporting period; activities planned for the following period; problems encountered and actions <br />taken to remedy them; list of meetings attended; and overall status. The Consultant shall <br />invoice according to the breakdowns as shown in Attachment "C" -Fee Schedule. <br />Field Equipment. The Consultant shall provide computer equipment, Personal Protective <br />Equipment (PPE) and identifiable vehicles for its personnel. The computer equipment shall <br />include tablets and cell phones. The PPE shall meet all current standards set by OSHA. <br />Page 1 of 1 Attachment A <br />
