3) A detailed breakdown of the cost of the change (necessary <br />only if the modification requires a change in contract <br />amount). The itemized breakdown is to include: <br />(a) list of materials and equipment to be installed. <br />(b) man hours for labor by classification. <br />(c) equipment used in construction. <br />(d) consumable supplies, fuels, and materials. <br />(e) royalties and patent fees. <br />(f) bonds and insurance. <br />(g) overhead and profit. <br />(h) field office costs. <br />(i) home office cost. <br />(j) other items of cost. <br />4) A revised schedule indicating the effect on the critical path <br />for the project and a statement of the number of days the <br />project may be delayed by the modifications. <br />2. A contract modification request is required for field changes. <br />a. Request must be made a minimum of two (2) weeks in advance of <br />performing the work affected. <br />b. Request for field changes will be submitted to the ENGINEER. <br />3. A contract modification request is required for all substitutes or deviations <br />from the Contract Documents. <br />4. ENGINEER will evaluate the request for a contract modification. <br />E„ If a contract modification is required, the OWNER will issue a Field Order or a <br />Change Order per the General Conditions. See Change Order form in Specification <br />Section 00 63 63. <br />1. Modifications to the contract can only be made by a Field Order or a Change <br />Order. <br />2. Changes in the project will be documented by Field Order or by a Change <br />Order. <br />3. Field Orders may be issued by the OWNER for contract modifications that do <br />not change the contract amount or contract time. <br />4. Any modifications that require a change in contract amount or contract time <br />can only be approved by Change Order. <br />a. CMR's and proposals issued by the CONTRACTOR in response to a <br />PCM will be evaluated by the OWNER. <br />b. If a Change Order is recommended, the OWNER will prepare the <br />Change Order. <br />C.
