<br />Guidelines for Completion of Standard Form 424 J <br />Other Prescribed Office of Management and Budget 'Forms J <br />and Crimin~l Justice Division SUItf!ested Application Format <br /> <br />Attachment M of the Uniform Grant and Contract Management Standards <br />contains forms and instructions for applying for all types of state and federal <br />funding. <br /> <br />Only two of the exhibits in Attachment M apply to criminal justice related <br />grants -- Exhibit M-3 for non-construction proiects (pages 11-23), and <br />Exhibit M-4 (pages 24-37) for construction projects. <br /> <br />The majority of applicants will be using exhibit M-3, and should refer to the <br />following instructions in completing the applications. Sections marked "Not <br />Applicable" (N / A) do not require any information from the applicant. <br /> <br />For clarity in referencing, each page in the application package is given a <br />CJD page number (e.g., CJD-10), to which these guidelines refer. <br /> <br />The federal form 424 (Pages CJD-l and CJD-2) included in this package <br />supersedes the corresponding form printed in th~ ,current edition of the <br />Uniform Grant and Contract Management Standards.;' The old form may not <br />be used. . <br /> <br />In all sections of the application J express amounts in whole dollars only. <br /> <br />Part I - Standard Form 424 (~s CJD-l throuJth CJD-2) <br /> <br />The Standard Form 424 (page CJD-1) will be the front sheet of the grant <br />application. <br /> <br />Section I - Applicant/Recipient Data <br /> <br />1. Enter date submitted to Council of Governments/Regional <br />Planning Commission or, for State Agencies: and Nonprofit <br />Organizations, enter date submitted to CJD. <br /> <br />2. a.
