Mark "New" if the project is not presently funded by CJD grant. <br />Mark "Continuation" if the project is presently funded by CJD <br />grant. <br />Mark "Revision" if the document relates to a grant presently <br />funded by CJD or to one that is pending CJD <br />approval/ disapproval. <br /> <br />9. The required information has been preprinted. <br /> <br />10. Enter the 5-digit code and the title (e.g., SF-AOl Crime <br />Prevention) that describes the program, ~nder' which the <br />application is submitted. That code and title appear in the <br />Program Description section of the Grant Application Kit. <br /> <br />11. Self-explanatory. Do not merely repeat the title ~ stated in item <br />#10. Briefly describe what the project will do. (Continue on <br />page 2, if necessary. ) <br /> <br />12. List the cities or counties to be served by this project. State <br />agencies, universities, and nonprofit organizations list <br />"Statewide" . <br /> <br />13. Start Date: Enter the date on which this grant will start. <br />Ending Date: Enter the date on which this grant period will <br />end. (Grants are usually approved for a period of 12 months.) <br /> <br />14.a. Enter information as requested. <br /> <br />14.b.
