Brady Environmental Services, Inc. <br />1.1.14 Licensing <br />1.1.14.1 The contractor shall be licensed through the Texas <br />Department of State Health Services for asbestos <br />abatement. All supervisors and workers involved <br />with the project (during prepping, pre-cleaning, <br />removal, etc.) shall be EPA accredited, Texas <br />licensed and/or registered, and shall have a current <br />medical evaluation and respirator fit tests in <br />accordance with the OSHA requirements of CFR <br />1926.1101. <br />1.1.15 Submittals <br />1.1.151 Pre-Job Submittals <br />The following items shall be at the job site for review <br />by the consultant prior to the notification start date: <br />a. EPA Training Certificates for all supervisors <br />and workers and current Refreshers. <br />b. TDSHS Licensing: <br />Abatement Contractor License <br />Asbestos Transporter License <br />Asbestos Supervisor <br />c. TDSHS Worker Registration <br />d. Medical clearance for each worker and <br />supervisor <br />e. Certificate of Insurance <br />1.1.15.2 Post-Job Submittals <br />The following items shall be delivered to the project <br />consultant within ten (10) days of the project's <br />completion: <br />a. Hazardous Waste Manifests <br />b. Project Job Log <br />c. Copies of all final laboratory analysis reports: <br />Personal sample results. <br />- - g - CS09055 <br />
