include the name of the laboratory that will do the blood lead testing and the <br />name and location of the laboratory that will test the personnel air monitors for <br />exposure assessment. The Plan must also address required training and <br />Certification of such training for all workers and the competent person. <br />4. Disposal of Debris Program: The Contractor will submit a written plan <br />detailing the procedures used to properly handle, store and test debris to <br />determine if it is hazardous. In addition, the program must identify the license <br />number of transporters and disposal facilities to be employed. The written <br />program must also explain how manifesting will be addressed. <br />5. TCEQ Notification: Contractor shall notify in writing the appropriate <br />TACB regional office and any local authorities having jurisdiction over <br />abrasive blasting activities of the blasting in writing in accordance with TCEQ <br />Rule 111.135. Prior to notification, contractor shall submit a copv to the <br />Engineer for approval. Per Rule 111.135, the notification must include: <br />(1) the location of the tank; <br />(2) the name of the abrasive blasting company; <br />(3) the weight percent of lead in the coating; <br />(4) the control methods to be used; and <br />(5) the expected hours of operation and scheduled start and finish dates. <br />TS35-8 4s9 <br />
